What to Expect
Here’s what to expect during your first visit to Mobility Plus Physical Therapy.
WHAT INFORMATION SHOULD I BRING?
When you arrive for your first appointment, please provide the following:
- Prescription for Physical Therapy
- Insurance Referral (if required)
- Insurance Card
WHEN SHOULD I ARRIVE?
Please arrive 15 minutes early to your initial appointment.
This will allow time to fill out required patient forms.
WHAT SHOULD I WEAR?
Please wear comfortable or loose clothing.
Shorts or sweatpants with a t-shirt is ideal for comfort.
You can change here if you prefer.
HOW LONG ARE APPOINTMENTS?
Appointments last between 45 and 60 minutes.
Please inform us if you need to leave early.

VERIFYING COVERAGE
We recommended contacting your insurance company prior to your first appointment to verify your coverage. You will need to follow the guidelines of your insurance company. In addition to a prescription for Physical Therapy, you may be required to obtain an insurance referral.
Please let us know if you are covered under workers compensation or a motor vehicle accident when you schedule your appointment. We will need to obtain your claim number, date of injury, adjuster’s name and phone number to verify benefits and obtain authorization. Private pay options are also available.
CO-PAYMENTS + DEDUCTIBLES
Patient co-payments and deductibles are required on the date of service. To prevent large balances, smaller payments can be made in advance, or when you receive an explanation of benefits showing the amount owed toward each visit. We accept cash, checks, and all major credit cards.
CANCELLATION POLICY
If you need to cancel or reschedule your appointment, please contact us at least four hours in advance so that we may attempt to fill your appointment time and to avoid a cancellation or no-show fee.
